EPFO Form 5A Display Guidelines 2025

Updated Guidelines
• Per EPFO (Employees’ Provident Fund Organisation) circular dated October 7, 2025 all establishments under EPF coverage have to display an extract of Form 5A.
• The extract of Form 5A has to be displayed at the company’s entrance. It should also be uploaded on the company website / mobile app (if available).
• The deadline for completion of the above is October 22, 2025.

What Must Be Displayed in the Extract of Form 5A
The extract should include the following key details:
• EPF Code.
• Registered name of the establishment
• Date of commencement of EPF coverage.
• Number of branches and address of the main branch.
• Regional EPFO office the establishment falls under.


Why is this important?
• Increases transparency and visibility for employees and visitors to verify the EPF registration.
• Helps EPFO’s field inspections by ensuring information is prominently visible.
• Non-compliance of the above may attract legal scrutiny or penalties under the EPF & MP Act, 1952.

Best Practices & Tips
• Regularly review and update the Form 5A details (if there is any change in ownership, branches, etc.).
• Maintain an internal compliance log with dates of display and uploads.
• Share the directive with HR, legal or audit teams so everyone is aware and aligned.
